At HMP Ashfield, an adult male sex offender prison based in Gloucestershire. we’re committed to delivering a safe, secure, and rehabilitative environment that supports the needs of prisoners and staff alike.  We’re looking for a dedicated and compassionate Social Prescribing & Wellbeing Specialist to join our team and make a real difference in the lives of offender - many facing complex social, emotional, and practical challenges.

 

About the Role


As a key member of a multidisciplinary team, you will assess individuals needs, co-create personalised wellbeing plans, and link them to non-clinical community services that promote health and resilience. Your work will focus on empowering people to take control of their health, build confidence, and overcome barriers like social isolation, unemployment, and lifestyle challenges. You will also support the delivery of our local drug strategy, ensuring individuals affected by substance misuse are guided towards appropriate interventions.

 

What You’ll Do

 

  • Conduct holistic assessments and develop tailored wellbeing plans.
  • Connect individuals with community activities such as physical exercise, mental health support, education, and creative therapies.
  • Provide ongoing encouragement and monitor progress to ensure lasting engagement.
  • Collaborate with healthcare professionals, social workers, and community partners to coordinate care.
  • Advocate for individuals’ needs and promote healthier lifestyle choices.

 

What We’re Looking For

 

  • Experience of working directly with people in a supportive, enabling, or community-focused role (e.g., health, social care, voluntary sector).

 

  • Experience of managing a caseload and providing 1:1 personalised support.

 

  • Experience of partnership working with statutory and voluntary/community sector organisations.
  • Strong understanding of social determinants of health.
  • Experience of supporting individuals affected by substance misuse, with knowledge of related services and interventions.
  • Excellent communication skills, empathy, and the ability to motivate individuals facing multiple barriers.
  • Commitment to equality, diversity, and inclusion.

 

Desirable

 

  • Experience in social prescribing or case management.
  • Knowledge of local health, social care, and voluntary sector services.
  • Mental health first aid or motivational interviewing training.
